Online Application

The Online Application allows you to set up your profile and pay immediately with a credit or debit card via PayPal. (You do NOT have to have your own PayPal account to use the PayPal interface.)

Before completing the online application, please review the "Member Code of Conduct" by selecting the link in the navigation bar. You will be asked to accept these terms as part of the application process.

After selecting the Membership Level, scroll down and click 'NEXT' in the lower right hand corner to proceed. All items with a red asterisk (*) are mandatory.

Short on time? Complete all items with a red asterisk (*) and login later to update your profile.

To pay by check or money order:
Complete the online application as per above and stop at the PayPal interface. You will receive an email confirmation that your application is pending. To help alert the AIWC Membership committee that you are sending a payment, please send an email to info@azintegrative.org with 'AIWC Membership' in the subject line.

Send your check or money order to: 3400 E. Speedway Blvd #118-350, Tucson, AZ 85716.

Please allow 14 business days for processing. When your membership is activated by AIWC, you will receive an email welcoming you to AIWC.

Business I - $180.00 (USD) Bundle (up to 8 members) Subscription period: 1 year No recurring payments Business I is for organizations, businesses, or group practices of five or move full-time employees (or equivalent).

Includes:

* Listing in our online Member Directory
* Opportunity to attend events at membership fee rate
* Opportunity to display AIWC logo on your website and at your place of business
* Other member benefits as described on Membership Information page
* Eligibility to participate in Member-to-Member discounts
* Option to list up to seven (7) additional individuals (beyond Primary Contact listed above) who may participate in Member-to-Member discounts and may represent your organization at Coalition events.

Business II - $100.00 (USD) Bundle (up to 4 members) Subscription period: 1 year No recurring payments Business II is for those with a private professional practice or organizations, businesses, or group practices of up to four full-time employees (or equivalent).

Includes:

* Listing in our online Member Directory
* Opportunity to attend events at membership fee rate
* Opportunity to display AIWC logo on your website and at your place of business
* Other member benefits as described on Membership Information page
* Eligibility to participate in Member-to-Member discounts
* Option to list up to three (3) additional individuals (beyond Primary Contact listed above) who may participate in Member-to-Member discounts and may represent your organization at Coalition events.

Student/Community Members - $40.00 (USD) Subscription period: 1 year No recurring payments Individuals not seeking to promote a professional practice, business, or organization.

Includes:

* Opportunity to attend events at membership fee rate
* Eligibility to receive Member-to-Member discounts
* Does not include listing in online AIWC Member Directory nor the privilege of displaying AIWC logo
